Leaders actively support BHICCI as core system transformation initiative that will impact organizational culture.

July – Oct 2016

  • Develop clinic/program level Charter aligned with BHICCI Charter
  • leader(s) communicates to HCO system the aim, goals, and outcomes of BHICCI, including alignment with other initiatives.

Oct 2016 – Feb 2017

  • HCO administrative and clinical leaders address barriers to implementation including space, equipment, integration with other clinic staff and functions, allocation of management and clinic staff time and resources

Feb-July 2017

  • Leaders track and share BHCCI outcomes identifying areas for spread to other HCO programs

July 2017 – July 2018

  • Leaders support testing and implementation of improvements across clinic or other HCO programs
Executive leaders address barriers to team hiring and team formation.

July – Oct 2016

  • Executive leaders address insure timely recruitment and hiring of BHICCI Team members

Executive leaders develop collaborative cross system relationships.

July – Oct 2016

  • Leaders work with other regional organizations to improve cross system collaboration through improving care transitions
  • Map key care transitions within/across organizations

Oct 2016 – Feb 2017

  • Develop and test policies, protocols and agreements to improve system/cross system patient care transitions

Feb-July 2017

  • Implement protocols and agreements to improve patient care transitions

July 2017 – July 2018

  • Track care transitions outcomes, continue to test/modify as needed. Design, test, implement additional care transiĀ­tions initiative(s)
